چکیده

(Intern Med 50: 173-174, 2011) (DOI: 10.2169/internalmedicine.50.4363) To the Editor We read with great interest the papers by Tomii and colleagues and Yokoyama and colleagues concerning the use of non-invasive ventilation (NIV) in managing life-threatening acute exacerbation of idiopathic interstitial pneumonia (1, 2). We recently reviewed the data of 30 patients suffering of idiopathic pulmonary fibrosis (IPF; n= 22) or idiopathic non-specific interstitial pneumonia (NSIP; n=18), according to the international consensus statement, admitted in our intensive care unit (ICU) for acute respiratory failure (ARF) between 01/01/1999 and 31/12/2009 (Table 1). We chose to associate the two patterns in the same

Interstitial lung diseases in the hospitalized patient

BACKGROUND Interstitial lung diseases (ILDs) are disorders of the lung parenchyma. The pathogenesis, clinical manifestations, and prognosis of ILDs vary depending on the underlying disease. The onset of most ILDs is insidious, but they may also present subacutely or require hospitalization for management. ILDs that may present subacutely include acute interstitial pneumonia, connective tissue d...

Rescue therapy with a pumpless extracorporeal lung assist device in a patient with acute interstitial lung disease and severe refractory hypercapnia.

Idiopathic interstitial pneumonia frequently causes severe pulmonary restriction that in turn makes mechanical ventilation difficult. We report the case of a 44-year-old woman who developed a refractory severe hypercapnic respiratory failure (P(aCO(2)) 281 mm Hg, pH 6.77) despite mechanical ventilation with high inspiratory pressure and PEEP. A pumpless extracorporeal lung assist device, Noval...

Why do patients with interstitial lung diseases fail in the ICU? a 2-center cohort study.

BACKGROUND Admitting patients with interstitial lung disease (ILD) to the ICU is controversial, due to their associated high mortality when they require invasive mechanical ventilation. We aimed to determine the risk factors for mortality in ILD patients requiring ICU support due to acute respiratory failure. METHODS An observational cohort study was performed in 2 chest diseases teaching hos...

High serum concentrations of autoantibodies to HSP47 in nonspecific interstitial pneumonia compared with idiopathic pulmonary fibrosis

BACKGROUND The pathological diagnosis of idiopathic interstitial pneumonias (IIP) by surgical lung biopsy is important for clinical decision-making. However, there is a need to use less invasive biomarkers to differentiate nonspecific interstitial pneumonia (NSIP) from other IIP such as usual interstitial pneumonia (UIP). Heat shock protein (HSP) 47, a collagen-specific molecular chaperone, is ...

Dyskeratosis Congenita Associated Non-Specific Interstitial Pneumonia

Dyskeratosis Congenita (DC) is a rare inherited disorder of ectodermal dysplasia. It consists of a classical mucocutaneous triad of abnormal skin pigmentation, nail dystrophy and leukoplakia. Pulmonary disease is seen in 10-15%. It is characterized by Idiopathic Pulmonary Fibrosis (IPF), or Idiopathic Familial Pulmonary Fibrosis (IFPF). Non-specific Interstitial Pneumonia (NSIP) has been report...
